About this office
Our Forest Hills office is on Yellowstone Boulevard at 69th Avenue, near 108th Street, on the north side of Forest Hills. Like our other offices it is AAAHC-accredited for office-based procedures, and ultrasound and urine, blood and STI testing are done on site, so most questions are answered at the same visit.
Getting here by subway and bus
The Q23 bus stops at the corner of Yellowstone Boulevard and 69th Avenue, steps from the door. The subway is a longer walk: 63 Dr–Rego Park and 67 Av (M and R trains) on Queens Boulevard are each about 20 minutes on foot, so many patients come by bus, car or taxi. If you are driving, allow time to find parking on the surrounding residential streets.
Languages
Dr. Guichard sees patients in English, French, Haitian Creole and American Sign Language. Stephanie Nguyen, FNP-BC, also sees patients in Vietnamese.
If English is not your first language, a professional medical interpreter is available in any language, in person or by phone. Tell us which language when you book.
Before your visit
Please bring a photo ID, your insurance card, a list of the medications you take, and any recent results or records that relate to your visit, such as a prior Pap result or an ultrasound report. For a first visit, arriving about ten minutes early gives us time to register you.
